Cart looks very much like an Audio Technica AT-6. Dont's know the headshell, though - sorry.

Thanks again. The cartridge looks correct but it appears to be glued or otherwise bonded to the headshell.
 
I took a picture of your photo and Google searched the photo.
Here is what I found. Looks just like yours.

Btw, I was curious and did a little catalogue search: Seems like the headshell might belong to the JVC/Victor/Nivico SRP-471E-5 - so it's indeed pretty likely, that the cart is a Victor MD-1004 labelled AT-6.

I have that headshell - It came on a Nivico turntable, the SRP 471E3. Even though it has a ten inch platter, it has an eddy current speed control, and a double idler system - the normal idler moves a fixed idler that is much deeper than any I've seen on a turntable - it looks like it might be a RTR capstan.
Anyone with one of these tables probably has noticed that the grommets in the motor/eddy current mechanism are done for. But AKer Tothness has very nicely made silicon replacements. I haven't had time to put mine in, but looking forward to hearing the table as it was meant to be.
