Hey, Keith, my coupla thoughts, most of which have already been made one way or another...
In descending order of WAF...
1. I like the idea of small, bookshelf-sized speakers on the TV stand, if the shelves are far enough apart, or lay the speakers on their sides. After all, this isn't your primary two-channel music rig, so you really won't be missing too much in imaging if they're that close together. If the shelves are too small to accommodate acceptable speakers, then a new stand might be in order...
2. Put small bookshelf sized speakers on very minimalist black stands on either side of the tv. That will get them oriented around the tv properly, although one would still be on the hearth, and the other to the left where the floor register is. It might look a bit odd with one on the hearth and one not.
3. Speakers up in each corner, painted to match the walls. Wouldn't be my first choice, just tossing out ideas.

I'm assuming you don't want to cut holes in the ceiling.
4. Sound bar and sub.
5. Move the couch back in front of the windows, and the tv on the long wall. Solves every problem, except, of course, for the fact that you don't want the couch in front of the windows.
There's just not a lot of room left of the fireplace, so if you want to keep it there some compromises are going to be unavoidable.
