2226B Bias Issue

I have a 2226B that I am trying to adjust, I've done a lot of these without this problem. When trying to adjust the bias I cannot get it at the required 18mv, it will max out at 2.5mv on both channels. The heatsink is hot to the touch after a few minutes. If anybody has a clue to what I need to check or look for please let me know..
I am going off the 2226B service manual for specs.

Is your meter reading .0025 or 0.250, hot indicates its higher than 0.018v not lower.

I have a Fluke MM and on both channels while playing with the resistor pot it will fluctuate between 0.0MVDC to 1.9MVDC, i cannot get it any higher than that

I've done more than 10 2226B's and never had this problem
 
Ah ok. Test the differential pair before the outputs, any time I've had bias issues one transistor is shot.
