4000 Quadradial Adapter Schematic

I have been searching for weeks for a schematic of the 4000 Quadradial Adapter. Even the eBay sellers don't have it. I really only want to see how the meters are setup and how the attenuator switch is wired.

Does anyone have the schematic or a scan of it?
