My contribution to this thread, I think, would be the award winning Command Records covers of their early days. The primary (perhaps only) artist/graphic designer was Josef Albers.

I've always liked this cover art, from one of Schulze's classic albums, Timewind (1975). I don't know what the story behind it is, and the music is purely instrumental, so no help there.
