Since this info doesn't seem to be readily available and I have one of the original instruction "manuals" (actually just a stapled packet), I figured I would upload it for anyone who needs it. Hope this helps someone out!

Yes thanks very much as well just in the process of setting up a McCurdy hh-12 which is an older version of a ch-12 with a Gray 216 tonearm.
Building a wooden plinth and this is just what i was looking for.
