KSW 12 subwoofer trouble

Hello, I am very new to speakers/home theater setup. I am trying to set up four old speakers I have inherited, prior to this I have only used 2ch setups for my PC. I have:
Yamaha RX V995 receiver
2x Klipsch Quartet QUR 00 main speakers
Klipsch KSC-C1 center speaker
Klipsch KSW 12 subwoofer (the troublemaker)

So far I have the center and main speakers working fine. The sub is plugged directly into the wall for power, and uses an RCA cable that splits in to two at the subwoofer end. When I turn the receiver on the sub turns on as well and sounds like it should work fine. I am using a record player connected at PHONO to the receiver to test things out. The sub makes no sound when I am listening to music, but occasionally (and almost always if I adjust the bass balance) will begin to make a tone that builds in volume until I crank the overall volume on the receiver to zero.

It was all working when I got the system, but I fear I may have done something to the receiver's settings while trying to familiarize myself that is keeping the sub from playing back.

Any troubleshooting advice or direction on how to set up the receiver correctly would be greatly appreciated! (please forgive me if this is on the wrong board)

Does the receiver have a sub preout? Do you have any other source to play with? Have you tried speaker level inputs?
 
I don't see anything in the manual or on the receiver that would indicate it does have a sub preout (learning what preout is from a cursory search). As of yet I do not have any other source set up, but if it's necessary I can take an RCA cable from my PC to use my phone or laptop as a source temporarily. By speaker level inputs, do you mean inputting the sub as a speaker (in the same manner the quartets are wired)? To describe the sub-receiver connection in more detail, I have a split RCA for R and L/LFE at the sub that joins, and then goes to a single RCA "external decoder input" that is labelled specifically for a subwoofer.
 
That Yamaha model does have a sub preout and main preouts. I would run speaker wires to the sub high level or speaker level input temporarily just to see if you level.
 
Sounds like a feedback loop. Do you have the same problem with other signal sources like a CD player or FM?
 
I got a chance to work this out yesterday. I realized that the "external decoder input" I was plugging the amp in to was not correct... there was another rca jack for the sub that no longer had any label on the back of the receiver. I switched it over and it works fine. I also got some more cable and was able to test this out on my tv and phone as sources and they are all sounding fine as well. This seems a trivial resolution but hey at least its solved.

That being said, I'm not sure what the role of external decoder inputs are but I reckon it won't be anything I get involved with anytime soon.
