Thank you for your suggestion. I am actually using a product called ThermoPad to electrically insulate the output transistors from the heat sink. They are made of silicone and fiberglass and appear to be working well.
Sorry, I should have posted here that I solved this problem. When I replaced Qm19,20,21, and 22, I swapped the NPN transistors with the PNP transistors. This caused all sorts of voltage reading craziness, although not any permanent damage. Swapping them back to their correct positions solve all of my problems. The amp is now running great!
