Are you looking for the black plastic rocker levers, or the actual switch itself? The actual switch is of the SPDT type and can be found almost anywhere. Don't know where to get the black plastic lever replacement.

The PAT-5 switches used round black pushbuttons rather than the black rockers of the PAT-4. The switches were made by either Alps or Noble; I can't recall which, this many years after last working on a PAT-5. You might check online parts sources such as Digi-Key, Newark or Mouser to see if any of them stock Alps or Noble switches that use a similar button.
 
C & K has a bunch of push button caps, check out any of their push button switch data sheets at Mouser. C & K even has the Hafler DH-101 switch caps that mechanically change colors when depressed.
