Probably not in this case but perhaps there were other clips, or such (screwdriver?), in proximity when it was opened. Anything placed near an open diaphragm should be checked for magneticity first.
Even the magnets on my Heil drivers attract ferrous objects and if they are small enough, and near enough, they will end up through the diaphragm.
