Please help me to identify this speaker and places to get repaired. I believe the speaker is from B&O .

The Lafayette branded drivers from Japan were sourced from various Japanese OEM suppliers and could be from Foster/Fostex or Coral. Such a driver would never be found in a B&O branded system and the Phillips dome tweeters were widely used by many speaker companies in the 1970's and 80's including B&O.

That little woofer may not be repairable or the cost may be prohibitive, but good luck.

Lafayette was an electronics chain similar to Radio Shack. They closed down in 1981. To my knowledge, no commercial speaker manufacturer ever used a Lafayette-branded driver in their speaker.
 
Identical(nearly) woofer to the 12HCW-1 used in the Minimus-1.

But, i don't think it's a Foster/Fostex.

Matsushita made a very similar woofer, same square frame with truncated corners.
